MOTORHOME MEDICAL

We provide D4 driver medicals for you to renew or apply for a C1 licence. You can use your C1 licence to drive a motorhome or heavier vehicles.

(Booked with at least 3 days prior working notice)

PLEASE BRING THE FOLLOWING TO YOUR APPOINTMENT:

1. D4 Medicals Form - Please fill in the grey section on page 1 of your form (left hand side) ‘Your details’ along with your name and date of birth at the foot of all other pages on the D4 form
2. Photo ID – Driving Licence/Passport
3. Your driving glasses and your glasses prescription if you wear glasses (not contact lenses) to meet the eyesight standard needed for driving
4. Your medication prescription
