PUBLIC SERVICE VEHICLE (PSV) MEDICALS
We provide D4 medicals for PSV, bus and minibus driving. At your medical appointment, our skilled Doctors will go through a questionnaire about your medical history. A blood pressure check and a vision test are also included in your appointment.

PLEASE BRING THE FOLLOWING TO YOUR APPOINTMENT:
- D4 Medicals Form - Please fill in the grey section on page 1 of your form (left hand side) ‘Your details’ along with your name and dateof birth at the foot of all other pages on the D4 form
- Photo ID – Driving Licence/Passport
- Your driving glasses and your glasses prescription if you wear glasses (not contact lenses) to meet the eyesight standard needed for driving
- Your medication prescription
